Age Limit: Candidates age should be between 21 to 32 years as on 15-07-2015. Age relaxations are applicable as per the rules.

Educational Qualification: Candidates should have passed +2 Science examination under Council of higher Secondary Education, Odisha or equivalent and Diploma in Pharmacy from any of the three Medical Colleges & Hospitals of the state/ any other recognised private Institutions duly approved by A.l.C.T.E. and examination conducted by the Odisha Pharmacy Board.

Selection Process: Candidates will be selected based on merit of given career assessment.

How to Apply: Eligible candidates send their application in the prescribed format along with self attested copies of relevant certificates to the Chief District Medical Officer, Sambalpur on or before 30-07-2015 by 05:00 PM.
